Serving with Children & Youth

Thank you for your interest in serving with the children and youth of Christ Church Madison! This is our central application to get you started on our Safeguarding process. Whether you're brand new to serving, or have reached your renewal window, this form is for you. (This registration system is intended for events, so please forgive the "attendee" language that we couldn't change in some places.)

This is the first step in the process. Once you submit this form, there may be additional information we need from you, including a background check authorization, acknowledgement of our Ministry Worker Handbook, etc. These will be clearly visible on the confirmation page and by email. Please complete those next steps ASAP!
